Thank you for attending the 2007 Scientific Assembly. If you would like to print a certificate of participation for the CME you completed at Assembly, please choose from the two options below. Add your name and the number of credits, then print and sign the form.

Note about CME Reporting for AAFP Members
If you are an AAFP member and submitted your CME credit electronically while at Assembly, those credits have already been added to your CME record.

You can still report Assembly CME, as well as check your CME record and print a copy, in the My Academy (Members Only) section of the web site.
